Sample Quality Assurance (QA) Statement of Work and Reporting Templates
In accordance with statewide policy, direct quality assurance oversight is provided primarily through the use of third party independent quality assurance contractors. When these professional services are required a competitive process that adheres to applicable state procurement rules is used. 
 
The sample quality assurance statement of work, appendices, and reporting templates attached below are collaborative work products developed in partnership with multiple state agencies.  The documents were developed to accelerate the QA procurement process and to ensure that consistent quality management and risk assessment processes and standards are applied to the oversight of each major IT project. 
 
Special thanks goes to agency staff working for the Oregon Department of Human Services (Office of Information Systems - Strategic Systems Initiatives), the Oregon Department of Transportation (Information Systems - Transportation Application Development) and assigned counsel from the Oregon Department of Justice (Business Transactions Unit).

Major IT Project Reporting Deadlines
For each reporting period, agencies with Major IT projects are required to submit the following reports to the IT Investment and Planning Section:
 
 
The submission dates for these reports through the end of 2008 are as follows:
  • Friday, December 14, 2007
  • Friday, February 15, 2008
  • Friday, May 16, 2008
  • Friday, August 15, 2008
  • Friday, November 14, 2008
